Helping you and your family reach and maintain good health is very important to Clear Rate. We are pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. Clear Rate remains committed to providing a competitive, cost-effective benefit program. Each year we take great care and detail in the benefit programs offered to employees.
Summary of Changes to the 2025-2026 benefit plans include:
All full-time employees working at least 30 hours per week are eligible to participate in the benefit plans offered by Clear Rate Communications.
Newly hired employees will be eligible following 30 days of active full-time employment. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children.

ENROLLMENT INSTRUCTIONS:
The Benefit choices you make during your open enrollment remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Life Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

United Health Care (UHC) will continue to be your medical and prescription carrier. You have the choice between three plans for the 2024-25 plan year. The three plan options include two PPO plans and an EPO plan. View the Providers & Networks for more information on how these plans networks differ from each other as well as the links below to locate participating providers.
If you are already enrolled in one of the Clear Rate UHC medical plans today and have not already done so, please be sure to register through the UHC member portal where you can access important information pertaining to your medical plans with UHC including participating providers, cost estimates for services, explanation of benefits/claims, and your accumulation amounts for deductibles and out-of-pocket costs. Look up your prescriptions by viewing the BCBSM Drug List. Use the Control F feature to type in the name of your prescriptions and determine which level the drug is and whether a Prior Authorization (PA) or Quantity Limits (QL) apply. You can also determine whether there may be lower cost alternatives for your prescriptions.

MAIL ORDER - Many drugs are available in a 90-day supply, rather than the 30-day retail supply. Typically, mail order drugs are subject to two-times the standard 30-day copays noted above, resulting in lower expenses for you. GOOD Rx - There are many tools online that you can use in order to save on prescription costs. One being www.goodrx.com, an online Rx database that allows you to find what pharmacy is the cheapest for your specific prescription. Additionally, you may be able to find a coupon that will greatly reduce your cost. It is important to remember that many of the coupons can only be used outside of your plan (insurance will not be applied and costs with the coupon will not count towards your deductibles and/or out-of-pocket maximums).
ASK YOUR DOCTOR – Make sure to ask if there are cost savings alternatives to the prescription they are providing. Many times, there are generic or different manufacturers that will save you money at the pharmacy.
The three medical plans function differently when it comes to networks and access. The PPO and POS plans provide out-of-network coverage for a variety of services, whereas the HMO only provides out-of-network coverage in the event of a true emergency. If you do seek care out of network, you will experience higher cost sharing in addition to possible balance billing. NEW! Blue Cross Online Visits is transitioning to Virtual Care through Teladoc Health
Members will continue to get the same urgent and behavioral health virtual care that they do today but with a new look and feel with better integration with other virtual care solutions they may be engaged in.
All eligible members will need to download the Teladoc Health app and create a new account with Teladoc Health. The Blue Cross Online Visits app will no longer be available as of 1/1/24.
Virtual Urgent Care
Have a virtual visit with a U.S. board-certified doctor for minor illnesses such as colds, sore throats, urinary tract infections and pink eye. Visits are available for adults and children 24/7.
Mental Health Services
Through the Mental Health option, you can connect with a licensed therapist or U.S. board-certified psychiatrist when you’re dealing with stressful situations or issues such as grief, anxiety and depression. Mental health visits require an appointment, but many therapists and psychiatrists have evening and weekend availability.

You have the opportunity to contribute to a variety of savings and/or spending accounts on a pre-tax basis which lowers your taxable income. View the information below for important details and guidelines for the various accounts offered by SAMPLEINC.
The Health Savings Account (HSA) allows you to set aside pre-tax dollars to pay for eligible medical, dental, and vision expenses. The POS Medical Plan is qualified for an HSA. By contributing to an HSA you reduce your taxable income, so you pay less in taxes — which saves you money.
HSA IRS Maximums are as follows and are on a calendar year basis. The maximums indicated below are a combined maximum for participant and any employer provided funds to the HSA. If you elect the HSA plan mid-year, maximums are prorated based on your effective date in the HSA plan.
$3,850 for a single contract | $7,750 for a two person or family
$1,000 "Catch Up" contribution for those age 55+

The Flexible Spending Account (FSA) allows you to set aside pre-tax dollars to pay for eligible medical and/or dental and vision expenses. The nice part about an FSA is you have access to the full annualized amount on day one!
The election you make cannot be changed throughout the year unless the change aligns with the qualifying event. For example, if you get married, you can increase your contribution, however, you cannot decrease or end your FSA for this type of life event.

The Dependent Care Reimbursement Account (DCRA) allows you to set aside pre-tax dollars to pay for eligible dependent care expenses. Qualifying expenses include daycare, nursery & preschool tuition, summer day camps, and more!
The election you make cannot be changed throughout the year unless the change aligns with the qualifying event. For example, you can increase your contribution if you add another member to your family through birth or adoption, however, you cannot decrease your contribution for this type of life event.

Clear Rate's dental plans provide comprehensive coverage to help you and your family maintain good dental health. The dental plans include both in and out of network benefits. Although your cost sharing is the same in and out of network, if you do go out of network it is likely that you will experience balance billing
When traveling out of the United States you also have access to the Passport Dental program giving you access to a worldwide network of dentists. English-speaking operators are available to help you access care. Mutual of Omaha Dental coverage outside of the United States is the same as Mutual of Omaha Dental coverage within the United States.

You have the choice of two dental plans - The Core Plan includes a deductible (for basic and major services only), no ortho coverage, higher cost sharing for your basic and major services, and a lower annual maximum - OR - The Buy-Up Plan. The Buy-Up plan has no deductible, richer benefits for basic and major services, orthodontia coverage is included for dependents (through age 19), and a higher annual maximum

Delta Dental of Michigan offers the nation's largest dental networks: Delta Dental PPO and Delta Dental Premier. Plus, you will not be balance billed if you see a PPO or Premier dentist - giving them added savings. If you do choose to visit a nonparticipating dentist you may be balance billed and may have to pay more.
When traveling out of the United States you also have access to the Passport Dental program giving you access to a worldwide network of dentists. Clear Rate offers vision coverage through Mutual of Omaha to help pay for eye exams, prescription glasses and contact lenses. Mutual of Omaha also provides discounts for Hearing Care and Lasik.
If you are already enrolled in one of the Clear Rate's Mutual of Omaha vision plan today and have not already done so, please be sure to register through the Mutual of Omaha member portal where you can access important information pertaining to your vision plan including participating providers, coverage information, and explanation of benefits/claims. If you are not currently covered, you will be able to register in the MOO portal as of your effective date of coverage.
Your vision network with Mutual of Omaha is the EyeMed network. You may use the eye care professional of your choice. However, when you visit a participating in-network provider, you receive higher levels of coverage. If you choose to receive services out-of-network, you may be required to pay that provider at time of service and will need to submit a claim to Mutual of Omaha for the applicable reimbursement based on the out-of-network reimbursement schedule.
Clear Rate understands the importance to you and your family to have strong financial protection. We provide all employees Basic Life/AD&D and Disability coverage at no cost to you. In addition to these benefits, we offer a variety of insurance you can choose from on a voluntary, employee paid basis so you can select what is best for your family. All of these coverages are provided to protect your income and out of pocket expenses.

A sudden accident or death can leave you or your loved ones in a vulnerable position. Clear Rate provides all eligible employees Basic Life and Accidental Death & Dismemberment (AD&D) coverage in the amount of $10,000 at no cost to you. Employees have the opportunity to enroll in additional Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage. The cost of Voluntary Life/AD&D is age rated, please see iSolved for your rate.
